Background and Rationale

Action Against Hunger (AAH) has been a key humanitarian actor in Somalia since May 1992 providing an array of services in Nutrition, Health, Food Security and Livelihood, Water Sanitation and Hygiene across 4 Regions; Bakool, Nugal, Banadir and L.Shabelle. The Medical, Nutrition and Health department is currently working with local partners and other agencies in providing treatment for malnourished children across the stabilization centres and the outpatient treatment programs spread across the 4 regions incorporating Infant and Child Feeding (IYCF) in all its interventions.

Somalia is currently experiencing one of its severe drought comparable with the 2011 drought emergency that led to death of over 200,000 people. In March 2017, the Somalia Federal Government declared the current drought a national disaster requiring speedy intervention from all the stakeholders. To this end, AAH has mobilized resources and teams are already on ground responding to the surging cases of malnourished children and AWD outbreak within Somalia. The AAH support is provided through 4 SCs, 9 OTPs and 5 Mobile Clinics spread across Somalia. Across all the OTP centres, AAH has put deliberate efforts to ensure that IYCF interventions are included in the CMAM programming for improved program outputs.

The approach to the IYCF intervention in Mogadishu comprise of health and nutrition education at both the fixed and mobile sites while supporting mother to mother support groups sessions across the sites. The groups which are supported by the CHWs and lead mothers have been an integral part in supporting adoption of appropriate IYCF practices including hygiene and sanitation.

This survey is intended to provide an understanding of the communities’ level of knowledge gained through various capacity building interventions including the existing indigenous and traditional knowledge, the attitudes and the practices and how these influence the health and nutrition status. The Survey results and recommendations will also help the nutrition and health stakeholders to design appropriate behavior change and communication interventions

Purpose of the TOR

The purpose of this TOR is to spell out the guidelines for the assignment that is aimed to carry out a study to assess the knowledge, attitudes and Practices amongst the communities in Mogadishu (Afgoye Corridor) regarding health, nutrition and hygiene practices and give a multisectoral overview and relevant recommendations that will inform and guide in appropriate interventions in the area.

Specific Objective

Specifically, this study aims to:

  1. To assess knowledge, attitudes and practice of appropriate Infant and Young Child Feeding and hygiene (IYCF) among mothers and caregivers;

  2. Determine breastfeeding and complimentary feeding indicators for children 0-23 months

  3. To determine factors that influence IYCF practices;

  4. To determine key challenges/barriers to appropriate IYCF practices and suggestions on how these can be overcome;

  5. To establish existing and other possible communication channels to support and sustain social and behaviour change towards optimal IYCF.

  6. To explore opportunities for integration and linkages of nutrition and health interventions to improve uptake of IYCF messages and behaviour change.

  7. Conduct 24 hour dietary recall and anthropometry to determine the actual food intake by child focusing on mothers/ caregivers of children 0-23 months

  8. Use KAP survey results to recommend key simple and achievable interventions that will address the identified issues to ensure appropriate practices

Methodology

The survey will employ both quantitative and qualitative approaches to establish the prevalence of minimum core IYCF practices focusing on the knowledge, attitude, and practices relating to maternal nutrition, infant and young child feeding, Water Sanitation and Hygiene (WASH) promotion interventions.

The survey shall further explore the communication approaches that drive social and behavior change towards adopting optimal MIYCN practices.

Main Tasks of the consultant

  1. Develop appropriate tools and instruments for gathering information;

  2. Present the KAP Methodology at AIMWG for validation

  3. Review appropriate documents (nutrition Survey reports, HIS reports etc.) relevant to the survey;

  4. Develop the survey plan; be responsible for preparation for the survey, including review of population statistics, calculating the sample size, selecting sample villages and respondents, developing an efficient survey schedule.

  5. Conduct KAP survey-Train data enumerators, ensure quality of data being collected

  6. Undertake consultations with key stakeholders and conduct field data collection; Focus group discussions, Key informant interviews etc

  7. Process and analyze the data, write and submit a draft report of the survey;

  8. Present preliminary findings at the Sub-cluster and cluster level, and present the final findings at AIMWG.

  9. Present the draft report of the findings in a stakeholder forum for comments and feedback

  10. Revise and submit a final copy of the survey report including recommendations to AAH.

Deliverables

Deliverables under this consultancy will include the following:

  1. A detailed inception plan showing how the consultant seeks to undertake the study. The report will include a clear methodology and data collection tools, analysis reporting details and a detailed work plan. This will be presented 4 days after the signing of the contract.

  2. Be present at NIWG for presentation of the KAP methodology and final findings A draft report detailing the survey findings and recommendations 7 days after completion of data collection activities in the field.

  3. A final report 14 days after completion of the field activities. .

  4. The final report shall be submitted as scheduled to the Head of Department in both electronic versions, MS Word and MS Excel, and 3 printed hard copies.

Proposed Time Frame

· The Consultancy is expected to last approximately 45 days including all the stages of the survey (desk review, enumerators training, data collection, support to data entry/analysis and report writing).

· The Survey is expected to begin on 24thJuly 2017.

Responsibilities of ACF

  • Provide guidance throughout the survey period.
  • Data entry/analysis shall be joint venture between consultant and AAH staff.
  • Logistic arrangements for all field travel
  • Approval of all deliverables including final sign offs for the purpose of making payments.
  • Provide transportation for official duties only in the field.
  • Provide enumerators
  • Provide return air ticket to the consultant.

Consultants Profile: Qualifications and experience required

The successful candidate shall:

  1. Possess documented experience in conducting KAP studies relating to IYCF.
  2. The consultant should preferably be a holder of a post graduate degree in public health, Nutrition, sociology, anthropology or related discipline. S/he must have strong analytical, negotiating, communication and advocacy skills.
  3. Have experience training and working with non-English speakers and people of low education levels.
  4. Have patience and experience in adult learning skills.
  5. Excellent interpersonal skills.
  6. Knowledge of the Somali language is desirable but not essential.
